How often should aquarium filter media has changed?

I know it is variable, but how I can know whether Mine has to be changed? EHEIM 2028 I have a boat with three different types of media. Almost sounds like you're not supposed to change?? I'm confused!

You never take the bio-media … this is what is the good bacteria and if it is removed, the tank must pass through a cycle again! carbon media should be changed when recommended, and it goes bad.
